This is the cast iron searing grate for the Primo Oval Large, and it is a specialist rather than a general-purpose grate. Its job is the first few minutes of a steak, a chop or a thick burger, when the surface either colours properly or it does not.
What decides that is how much heat is available at the exact point where meat touches metal. Cast iron holds a great deal of heat relative to its size and releases it slowly, so the bars do not cool off the instant something cold lands on them. Thin grate bars do, and the moment they drop below searing temperature the meat starts releasing moisture and stewing in it. The heavier grate simply keeps the contact points hot long enough to finish the job.
This grate belongs on the Primo Oval Large ceramic charcoal grill. Yes. This is the Oval Large version and the Large is the grill it belongs in. Primo makes the same grate for the other Oval sizes, so if you have more than one grill you will need the one named for each. There is no universal Primo searing grate.

Not at the same level. Both need to sit over the fire, so on a single tier it is one or the other. The workable pattern is to sear first while the fire is hottest, lift the grate off with gloves, and put the griddle down for whatever comes next.
Give it a good while over a settled fire. The benefit of cast iron comes entirely from heat it has already absorbed, and absorbing heat takes time. Put it on when you light or shortly after, and use the wait to bring the meat out and dry the surface, which matters as much as the grate does.
